Family Based Mental Health Services
Unfortunately, our Family Based Mental Health Services program closed in August 2023.
Though we no longer provide this service, we would be happy to direct you to another agency who can help.
Family Based Mental Health Services are therapeutic counseling services aimed at helping parents care for their children and teens who have serious emotional concerns at home and to reduce the need for out-of-home placements. FBMHS provides treatment using a team approach. Services are provided in the family’s own home and community.
Sometimes families need a hand.
We’ve all heard it said that it takes a village to raise a child, and it helps when your village includes professionals who are trained in helping parents and their children overcome behavioral and emotional issues that cause stress and disruption in the family. The goal of FBMHS is to help the family strengthen their communication and relationships and learn to cope and function effectively.
Who is eligible to receive FBMHS?
- Characteristics of children and youth (ages 0 to 21) who may qualify for services could include:
- Emotional and/or behavioral issues
- Diagnosed with a mental health disorder
- At risk for out-of-home placement (for example: partial or inpatient hospitalization, residential treatment, juvenile facility, foster care)
- At risk of change in school placement due to behaviors
- Child must be enrolled in medical assistance
- Caregiver must agree to actively participate in services
What do services look like?
Following a referral, families receive an intensive 30-day family assessment to determine the goals and needs in their treatment plan. Services include family therapy, individual/child therapy, sibling and parenting support sessions, case management, support in seeking and maintaining community services, and crisis prevention and intervention. Services are delivered by a team of two therapists in the child’s home, school, and/or community. From the start of FBMHS, the team helps prepare the family for discharge planning/aftercare services. The number of hours provided are based on the needs of the child and family, with a minimum of two sessions provided weekly. 24 hour crisis prevention is also provided.
How do I get FBMHS started for my child?
- Obtain a recommendation from the child's physician, psychiatrist, or psychologist through a script or evaluation.
- Choose your preferred provider.
- The insurance company (PerformCare) review recommendation and, if approved, sends a referral to your chosen provider.
- Provider contacts you within 24 business hours to initiate your referral intake appointment.
